'5 in 5' Five Tips to Improve Clinical Care for Diverse Patient Populations: An Online Activity

Hi there and welcome to 5 and 5, 5 tips to improve clinical care for diverse Patient Populations. This activity is supported by educational grants from Amgen, Galderma, Insyte, Leo Pharma, Lilly and L'. Oreal. My name is Dr. Susan Taylor and it's a pleasure to be joined by my colleague, Dr. Mona Shariari today. So let's get into it. Dr. Taylor, do you have any tips on how to create an office space or clinical environment that's inclusive? Oh, absolutely. So I think it's really important that we have paintings or posters on the wall that have images of diverse individuals. As important as to hire staff who are diverse. We should also ask our patients, what's your preferred name? Make sure your staff does that. Let's have name badges that are inclusive. Could say he, her, him or they. And finally, it's important to provide pamphlets in several languages, not just English. Think about Spanish, think about Portuguese or even Arabic. Those are great tips. I think we also want to consider that sometimes we may not see eye to eye with our patients, so we should respect and accept the differences and avoid ethnocentricity. I always say you want to check at the door any preconceived assumptions, notions that you may have, because what you think you might know about a person and that person's background may not apply to the individual that's sitting in front of you. Absolutely. So the tip here is ensure that your office space communicates inclusivity and diversity and have a culturally sensitive approach to each patient. So we know there are specific challenges in examining skin color and assessing disease severity. Any tips? Well, for many of our diseases, particularly our inflammatory diseases, severity is based on erythema or redness. And often that's a challenge to appreciate in our skin of color population. So there's several things we can do. First, we should always have bright lights in our examination rooms. We can also apply pressure to the skin either with our fingers or glass slides. We could see blanching. Next, let's say the erythema is on the extensor surface of the arm. You might want to turn the patient's arm over, look at the flexor surface and compare the two areas. You can often appreciate erythema when you compare areas. And finally, you can ask the patient if they think their skin is red. The patient will know. So that's really a simple, foolproof tip. I think that's an excellent tip and like you said, very easy to incorporate into day to day clinical practice. A couple of other tips that I like to use is I fully undress my patients and I try to use an area of normal skin as a compass to assess those areas that are not normal. And in individuals from different cultural backgrounds, maybe they have a hijab Islamic garb. You want to make sure you ask permission before you ask them to remove those aspects of their clothing because it may be culturally problematic. Also, some diseases have edema induration and using side lighting with your dermatoscope can be very helpful to better assess the elevation in the skin. But if that's not obvious, I just close my eyes and touch the skin. If I cannot tell where the lesion starts or stop, it's most likely post inflammatory changes that I'm dealing with. Yeah, you know, post inflammatory hyperpigmentation can easily be confused with, with active disease, certainly. So just to summarize the tip here, we have to recognize that the current methods of assessing disease severity may underestimate disease severity in melanin rich skin. So be more meticulous when examining skin of color patients. So it's really important for us to be assessing disease severity accurately so that we can ensure our patients are offered some of the newer and emerging therapies that can offer them disease control over time and avoid delays to these systemic therapies. This is a very exciting time for dermatology. There are so many therapeutic options for our patients and new ones on the horizon. All we have to do is ensure that our patients are diagnosed accurately and that they understand the nuances of the various medications that are now available. Now we know that historically skin of color patients have been underrepresented in dermatology clinical trials. As someone who does clinical trials, Dr. Taylor, why does this matter? Well, I think it's critically important. So if we prove that a medication or device is safe and effective for one group, you can extrapolate that it's going to be safe and effective for another group. So if you have a study that is representative of only one race or ethnicity, it doesn't represent our real world patients. Now, I think the great thing is that the FDA now requires know all the makers of various medications who undergo clinical trials for an NDA to have a diverse patient population. And I think that's a really important step. But it's not enough to just increase the numbers of patients from various backgrounds in our trials. We really need to understand why they're not participating. That might mean bringing on board new clinical trial sites, new investigators that look like the patients we're trying to recruit, understanding the social determinants of health that may preclude inclusion into those clinical trials. But thankfully, in recent years, the efforts that have been made to increase diversity in clinical trials is making some headway. We have some post hoc analyses, some prospective analyses, but I think the future is bright. I do, too. So another tip to keep in mind. Let patients know that the safety and efficacy of therapies are being studied in diverse populations, and the FDA is prioritizing diversity in clinical trials. Now we know that beyond objectively obtaining clinical trial data in patients with skin of color, the unseen scars and medical mistrust can oftentimes impact the desire to embark on a new, innovative therapy. How can we best address this? Well, you know, establishing trust is critical. It doesn't occur after just one visit. It's done over time. So when we have a new innovative therapy that we're so excited about, I think we have to step back. It's important during the first visit to provide the full range of therapies that are available and try to present all of the options equally. I try to give data on what's most effective, and then I ask the patient, what do you think? What do you feel most comfortable with? Often with our skin of color patients, they tend to be a little bit more conservative. They don't necessarily want what's new. They feel that they're being experimented on. So let's say we'll start with an older therapy. Then I have the patient come back at regular intervals. I then reintroduced into the conversation if they're not improving. You know, I don't think that you're getting the results that you are looking for. So let's talk about some of the other medications that are available. And at that point, I think they're more receptive and open to hearing about those new, latest jugs. So with our inflammatory disorders, it could be a biologic or it could be a JAK inhibitor. So, and I think the most important point is that building trust takes time. So it may take one visit, it may take five. So as clinicians, we need to be patient with the individual that's sitting in front of us. But it is very reassuring when we can tell them, you know what, 20% of the patients that were studied looked just like you. And that can go a long way in terms of solidifying the trust that we're trying to form. So to summarize the tip here, building trust takes time. Give patients the full range of treatment options and through shared decision making, ensure patient goals are prioritized. What are some tips on how to celebrate our patients from a variety of ethnic and religious backgrounds and even varying sexual orientations. Well, you know, it can be really simple things when a lot of times patients think they're the only one with a certain disease. So it's quite meaningful to them when you can reach on your shelf and pull down a textbook or an atlas with skin of color images, images of people with the same disease who look like them. And in a sense that's a way of celebrating them and celebrating their pigment. I think it's also important that the sources and resources that patients use, and it's often Google, also display a wide range of images of patients or people who look like our patients. If they are looking for additional information, we can guide them to other websites like Skin of Color Society, like the American Academy of Dermatology. They have a wealth of information with many diverse images of patients with a whole variety of skin diseases. And also a couple of other tips. Some non threatening ways to ask about ethnicity, because sometimes that can be a taboo topic. One thing I like to ask my patients is what's your ethnic background? Because there are certain conditions that can present differently based on where you originate from. And sometimes that kind of helps those shapes shields go down and they're more likely to respond or even you want to utilize an interpreter when it's appropriate so the patient feels heard. So our final tip, our approach to patients should help celebrate them for their own unique selves and help them feel comfortable in the skin they are in 100%. So let's summarize these five tips to improve clinical care for diverse patient populations. Number one, ensure that your office space communicates inclusivity and diversity and have a culturally sensitive approach to each patient. Recognize that current methods of assessing disease severity may underestimate disease severity in melanin rich skin. So be more meticulous when examining skin of color patients. Let patients know that the safety and efficacy of therapies are being studied in diverse populations. And the FDA is prioritizing diversity in clinical trials. Remember that building trust takes time. Give patients the full range of treatment options and through shared decision making, ensure patient goals are prioritized. And finally, our approach to patients should help celebrate them for their own unique selves and help them feel comfortable in the skin they are in. And with that, I offer you to take the test. WEDNESDAY, Feb. 11, 2026 (HealthDay News) -- For women aged 45 years or older taking spironolactone for dermatologic conditions, the incidence of hyperkalemia is 10.1 percent and is increased for older patients and those with predisposing conditions, according to a study published online Feb. 6 in the Journal of the American Academy of Dermatology.Samantha Gregoire, from Brigham and Women's Hospital in Boston, and colleagues examined the incidence of hyperkalemia in women aged 45 years and older taking spironolactone for dermatologic conditions in a retrospective cohort study. A total of 1,197 medical records were reviewed; 398 cases were analyzed.The researchers found that the incidence of hyperkalemia was 10.1 percent and was more often seen in older patients (22.4 versus 7.9 percent for those aged 65 years and older versus 45 to 64 years) and among those with one or more predisposing comorbidities (14.7 versus 7.3 for medically complex versus healthy individuals). The least affected were healthy women aged 45 to 64 years (6.3 percent), while the highest incidence was seen for patients aged 65 years and older with one or more predisposing comorbidities (28.1 percent). In general, hyperkalemia was mild and asymptomatic (97.5 and 85 percent, respectively) and did not affect clinical management (62.5 percent)."Identification of hyperkalemia on routine laboratory monitoring infrequently impacts clinical management or patient outcomes," the authors write. Hyperkalemia Risk Explored in Women Aged 45 Years and Older Taking Spironolactone

WEDNESDAY, Dec. 3, 2025 (HealthDay News) -- Both a lower-cost, ambient-light, nonpolarized dermatoscope and a higher-cost, illuminated, polarized dermatoscope enable patients to perform dermoscopy and receive teledermatology recommendations, according to a study published online Dec. 3 in JAMA Dermatology.Deonna M. Ackermann, M.P.H., from the University of Sydney, and colleagues compared a lower-cost ambient-light, nonpolarized dermatoscope to a higher-cost, illuminated, polarized dermatoscope for patient-performed teledermoscopy following early-stage melanoma treatment. The study included adults previously treated for early-stage melanoma in the patient-led surveillance arm of the MEL-SELF trial.Of 251 participants, 175 received a teledermatologist management recommendation for baseline images (polarized: 71.9 percent; ambient light: 67.5 percent). The researchers found that the proportion who received at least one recommendation was similar between the groups by 12 months (81.3 percent for polarized; 76.4 percent for ambient light). A per-image analysis showed more polarized images than ambient-light images were reportable and supported a recommendation (95.0 versus 91.1 percent). More positive feedback was provided for polarized images by teledermatologists, with blurriness (14 versus nine) and poor lighting (six versus zero) occurring more often with ambient light images. Patient usability ratings were similar (moderately/very easy, 39.8 versus 36.6 percent); more image quality issues were reported by ambient light dermatoscope users. When Amy Jones, PA-C, first stepped into emergency medicine, she didn’t yet know her calling would lead her to dermatology, and ultimately to building something new for her peers.After 15 years in dermatology and medical affairs, Jones has become one of the driving forces behind PANP360, a new learning hub and community designed by and for advanced practice providers in dermatology. Her vision: to create a space where PAs and NPs can find comprehensive education, clinical confidence, and a sense of connection that extends beyond CME.“If we don’t take care of ourselves, we can’t take care of patients,” Jones says. “Helping people master their craft so they feel confident in the care they provide—and also learn how to be well-rounded humans—is how we prolong our ability to see patients and live our lives to the fullest.”<h4>Amy’s Story from the ER to Dermatology</h4>What originally drew you to dermatology? I had practiced in emergency medicine, and that was very procedural-based and fast-paced. I knew after doing that for a few years that it was a young person’s game; it’s hard on the body and the lack of sleep is rough. I wanted that broad foundation of treating patients, but I knew it wasn’t a long-term fit for me.One of my PA school friends said, “You should look at derm. Everyone who does derm is really happy.” I started shadowing a PA near my home, and he was great. He let me learn the ropes from him. When their practice had an opportunity to add a PA, they offered it to me. That was a huge blessing.What changed for you once you transitioned into dermatology?&nbsp; I knew dermatology was going to be good, but I didn’t realize what was missing in the ER—the patient connection. In emergency medicine, you never have continuity; you see a different patient every time.Once I started having that in dermatology, it changed my life. I felt like I was part of something. I was able to recognize my patients, ask about their lives without reviewing their chart, "Your child graduated? Where did they go to college?” Those types of things really made me feel connected. That’s what dermatology did that I really loved.<h4>The Future of Care: PA and NP Leadership</h4>How have you seen the role of PAs and NPs evolve in dermatology?&nbsp; Dermatology is underserved, and I’ve really seen PAs and NPs fill that gap, especially in more rural areas. They’re setting up clinics, sometimes practicing independently or with collaborating physicians available by phone. That’s something that’s definitely changed over time.PAs and NPs are the future of medicine—definitely the future of dermatology. They’re a workforce trained to take care of patients at an advanced practice level, and that need is only going to keep growing.<h4>Building PANP360 Starts with a Holistic Approach</h4>What makes PANP360 different?&nbsp; I really want to focus on holistic healthcare providers who can take holistic care of patients. WEDNESDAY, Nov. 19, 2025 (HealthDay News) -- For patients with keloids, intralesional triamcinolone acetonide (TAC), intralesional bleomycin, and intralesional radiofrequency ablation (RFA) are all effective, with the greatest efficacy seen for TAC, according to a study published online in the November issue of SKIN.Aarti Singh, from Indira Gandhi Medical College in Shimla, India, and colleagues compared the efficacy and side effect profiles of intralesional TAC, intralesional bleomycin, and intralesional RFA for management of keloids in a randomized trial. A total of 117 patients were enrolled and randomly assigned to receive TAC, bleomycin, or RFA (groups A, B, and C, respectively); nine patients were lost to follow-up. The Patient and Observer Scar Assessment Scale (POSAS) was used to assess treatment efficacy.The researchers found that compared with baseline, all three groups had significant reductions in POSAS scores. Therapeutic effectiveness was 100, 80.6, and 72.2 percent in groups A, B, and C, respectively. Seventy-two percent of patients in groups A and B and 83.3 percent in Group C had side effects. During the eight-week follow-up after treatment completion, no recurrence was observed."TAC emerged as the most effective therapy, followed by bleomycin and RFA. While RFA showed lower efficacy and higher side effects, it holds promise as a noninvasive alternative, particularly with protocol optimization," the authors write. WEDNESDAY, Nov. 5, 2025 (HealthDay News) -- Patients with dermatomyositis who use immunostimulatory herbs before disease onset have lower positivity rates of myositis-associated autoantibodies (MAAs), myositis-specific autoantibodies (MSAs), and antinuclear autoantibodies (ANAs), according to a research letter published online Nov. 5 in JAMA Dermatology.In a retrospective cohort study, Xiwei Yang, from the Perelman School of Medicine at the University of Pennsylvania in Philadelphia, and colleagues examined whether commercial autoantibody testing results differ for patients with dermatomyositis that developed with versus without prior intake of immunostimulatory herbs. Positivity rates of MAAs, MSAs, and ANAs were compared for patients with versus without preonset herbal exposure; any herbal intake that lasted until three months before onset was considered relevant.The researchers found that 36 (13 percent) of 286 patients used immunostimulatory herbs prior to developing dermatomyositis symptoms. The median time was 12 months from first herbal use to dermatomyositis onset. The most commonly taken herb was spirulina (22 of 36 [61 percent]). Compared with the nonherbal intake group, the MSA positivity rate was lower in the herbal intake group (22 versus 51 percent), as was the MAA positivity rate (13 versus 32 percent). TUESDAY, Nov. 4, 2025 (HealthDay News) -- Two human leukocyte antigen (HLA) class I alleles are independently associated with an increased risk for allopurinol-induced severe cutaneous adverse reactions (SCARs), according to a study published online Oct. 29 in JAMA Dermatology.Chelsea N. Campbell, from Vanderbilt University Medical Center in Nashville, Tennessee, and colleagues examined the association of HLA class I and II among U.S. patients who received a diagnosis of allopurinol-induced Stevens-Johnson syndrome and toxic epidermal necrolysis or drug reaction with eosinophilia and systemic symptoms compared to allopurinol-tolerant and population control participants. Participants were identified from the Vanderbilt University Medical Center biobank, which includes 94,489 individuals with imputed HLA class I and II typing.The genetic association study included 16 patients with allopurinol-induced SCAR and 160 allopurinol-tolerant control participants. The researchers found independent associations for two HLA class I alleles with an increased risk for allopurinol-induced SCAR: HLA-B*58:01 and HLA-A*34:02 (odds ratios, 28.0 and 20.6, respectively). There were no HLA class II alleles that met the Bonferroni-corrected P &lt; 0.05 level of significance."We found a new genetic association, HLA-A*34:02, which appears very important as a risk factor in U.S. allopurinol severe cutaneous adverse reactions patients," coauthor Elizabeth J. Bell, M.D., from the Indiana University School of Medicine in Indianapolis, and colleagues compared scar appearance and surgical complications with a one-time application of HCD for one week after excisional surgery versus daily petroleum ointment in a randomized clinical trial. A total of 146 adult patients undergoing standard excision or Mohs micrographic surgery with linear bilayered repair were randomly assigned: 72 and 74 to HCD and petroleum ointment, respectively.The researchers found that for overall appearance, surgeon and patient ratings were clinically comparable. The difference in mean Visual Analog Scale rating between HCD and petroleum groups was –0.40, –0.08, and –0.09 at seven, 30, and 90 days, respectively. Higher, but not statistically significant, rates of adverse events were seen for the HCD group, including postoperative bleeding, wound dehiscence, and surgical site pain. Postoperative antibiotics were not required by any patients. A higher proportion of patients in the HCD group versus the petroleum ointment group rated the HCD as convenient or extremely convenient (86.9 versus 46.8 percent) and comfortable or extremely comfortable (73.8 versus 48.3 percent)."The clinical decision to use HCD versus daily petroleum ointment should balance cost and patient preferences for convenience and comfort with their risk of postoperative complications," the authors write. FRIDAY, Oct. 17, 2025 (HealthDay News) -- For people with HIV (PWH), the incidence of dermatologic disease (DD) has decreased over time but still remains prevalent, according to a study published online Oct. 14 in the Journal of the American Academy of Dermatology.Yagiz Matthew Akiska, from The George Washington University in the District of Columbia, and colleagues characterized the prevalence, incidence, and risk factors of DD among PWH in a longitudinal cohort study involving 11,738 adults enrolled in a cohort between 2011 and 2023. DDs were categorized as infectious dermatoses, inflammatory dermatoses, or cutaneous malignancies (CM).The researchers found that 49.4 percent of the participants had one or more dermatologic diagnosis, with infectious conditions the most common (41.4 percent). From 2011 to 2024, there was a decline in incidence across all categories of DD, including infectious (463 to 41), inflammatory (306 to 62), and CM (31 to six) cases per 1,000. The odds of CM were more than sixfold greater among cisgender females. TUESDAY, Aug. 12, 2025 (HealthDay News) -- Most individuals living in underserved communities believe that dermatologic side effects (DSEs) from anticancer therapies (ACTs) occur at high rates, according to a study published online in the August issue of the Journal of Drugs in Dermatology.Nikita Menta, from George Washington University in Washington, D.C., and colleagues evaluated knowledge and perceptions of ACT (medication or radiation)-related DSEs in a medically underserved community. The analysis included survey responses from 77 individuals attending two health fairs.The researchers found that 21 percent of respondents were previously treated for cancer. Overall, most respondents believed that ACTs cause hair loss (52 percent overall versus 31 percent of respondents previously treated for cancer), dry skin/rash (47 versus 50 percent), and nail changes (41 versus 31 percent). Respondents most frequently reported the DSEs that would make them possibly or definitely not undergo treatment were permanent hair loss (33 percent overall versus 13 percent of respondents previously treated for cancer), temporary eyebrow/eyelash hair loss (27 versus 13 percent), and permanent nail discoloration (24 versus 13 percent). Among patients who were previously treated for cancer, half did not visit a dermatologist during cancer treatment."These data underscore the need for improved DSE management," the authors write. Luo, from the Yale School of Medicine in New Haven, Connecticut, and colleagues examined the reliability of the ISS in individuals with Fitzpatrick IV to VI skin types in a cross-sectional study. Fourteen dermatologists used the ISS to rate 94 test photographs, 47 each from individuals with Fitzpatrick I to III and IV to VI skin types. Four body sites were chosen (upper back, legs, upper arm, and dorsal feet) due to limited comprehensive, high-quality photography among individuals with Fitzpatrick IV to VI skin types. Sixty-one patients with ichthyosis were represented in the 94 photographs. Interrater reliabilities were determined using intraclass correlation coefficients (ICCs).The researchers found that scale and erythema ICCs varied from 0.97 to 0.99 and from 0.91 to 0.95, respectively, among individuals with Fitzpatrick I to III skin types. The corresponding ICCs varied from 0.97 to 0.99 and from 0.90 to 0.94 among those with Fitzpatrick IV to VI skin types. FRIDAY, June 20, 2025 (HealthDay News) -- The U.S. Food and Drug Administration has approved Andembry (garadacimab-gxii) as the only treatment targeting factor XIIa for prophylactic use to prevent attacks of hereditary angioedema (HAE).According to the <a target="_blank" rel="noopener noreferrer" href="https://www.mayoclinic.org/diseases-conditions/hives-and-angioedema/symptoms-causes/syc-20354908">Mayo Clinic</a>, angioedema is "a reaction similar to hives that affects deeper layers of the skin. It can appear with hives or alone." Symptoms include welts that form in minutes to hours; swelling, particularly around the eyes, cheeks, or lips; and some mild pain.The approval of Andembry is for use in adult and pediatric patients aged 12 years and older and is self-administered with once-monthly dosing delivered in 15 seconds or less via an autoinjector with a citrate-free formula. Andembry targets factor XIIa, a plasma protein that plays a key role in attacks of swelling in people with HAE.The approval is based on results from a phase 3 study. Findings showed that 62 percent of Andembry-treated patients remained attack-free throughout the six-month study period. Andembry reduced HAE attacks by a median of more than 99 percent compared with placebo. WEDNESDAY, June 11, 2025 (HealthDay News) -- Use of three-dimensional total body photography (3D TBP) can diagnose general skin diseases with high accuracy, according to a study published online June 4 in the Journal of the American Academy of Dermatology.Sebastian Sitaru, M.D., from the Technical University of Munich School of Medicine and Health in Germany, and colleagues examined the value of 3D TBP in general dermatology and examined patient acceptance in an observer agreement study of the diagnoses of 294 patients presenting with skin changes obtained in face-to-face visits versus diagnoses obtained by retrospective analysis of 3D scans with no face-to-face patient visits. Questionnaires were used to evaluate patients' views on 3D TBP.The researchers found that across a broad range of diagnoses in a diverse patient group, primary diagnoses obtained by two senior dermatologists independently reviewing 3D TBP scans matched in 68.4 and 81.1 percent of cases, respectively. WEDNESDAY, May 14, 2025 (HealthDay News) -- A robust, deep learning model has been developed for the classification of melasma severity, according to a study published online April 16 in Clinical, Cosmetic and Investigational Dermatology.Jun Zhang, from Wuhan First Hospital in China, and colleagues developed an artificial intelligence-assisted real-time melasma severity classification framework based on deep learning and 1,368 anonymized facial images, obtained from clinically diagnosed patients. Six convolutional neural network architectures were trained and evaluated after image preprocessing and Melasma Area and Severity Index-based labeling, and model performance was assessed.The researchers found that the best performance was achieved by GoogLeNet, with accuracy of 0.755 and an F1-score of 0.756. Across severity levels, area under the curve values reached 0.93, 0.86, and 0.94 for mild, moderate, and severe, respectively. WEDNESDAY, April 16, 2025 (HealthDay News) -- In a special communication published online April 16 in JAMA Dermatology, recommendations are provided for the use of digital photography by dermatologists, with special considerations for skin of color.Madison Grinnell, M.D., from the University of Nebraska Medical Center College of Medicine in Omaha, and colleagues examined best practices for capturing clinically accurate dermatologic digital images, emphasizing techniques to improve photography portraying individuals of color, dermatologic conditions in skin of color, and pigmentary disorders.The authors note that historically, different kinds of photographic technologies have calibrated color correction using participants with light skin, creating inaccurate photographs among those with skin of color. Royal blue backgrounds are often preferred in photographing darker skin, as this color offers increased contrast, without resulting in aberrant hues. When possible, soft diffuse lighting, such as that emitted from an attachable ring light, should be used. Other aspects of photography should be standardized in the clinical setting to include a fixed distance from the patient and dedicated photography space. Specific lighting techniques, such as cross-polarization, may be used in accurately capturing erythema, inflammation, and pigmentary alterations. WEDNESDAY, Feb. 26, 2025 -- Dermatomyositis (DM) and clinically amyopathic DM (CADM) are often misdiagnosed, according to a research letter published online Feb. 26 in JAMA Dermatology.Noting that patients with DM often experience misdiagnosis, Aaron Bao, from the Johns Hopkins University School of Medicine in Baltimore, and colleagues identified common dermatologic mimics of DM, compared misdiagnosis rates between DM and CADM subtypes, and characterized potential implications of misdiagnosis. Adults with confirmed DM or CADM diagnosis and one or more Johns Hopkins dermatologic visits between 2005 and 2023 were included in the study.The analysis included 260 patients: 86 and 174 with CADM and DM, respectively (33 and 67 percent); 125 patients had inflammatory skin disease (ISD). The researchers found that 58 percent of those with ISD received an ISD diagnosis before DM or CADM diagnosis and 36 (29 percent) were misdiagnosed. Seventy-two percent of those with misdiagnosis received their skin evaluation by dermatology. Misdiagnosis rates were higher for the CADM versus DM cohort (44 versus 21 percent), with longer median diagnostic delays (236 versus 114 days). The most prevalent ISDs were unspecified dermatitis and eczema (22 and 19 percent, respectively). The highest misdiagnosis rates were seen for cutaneous lupus erythematosus and eczema before DM or CADM diagnosis (67 and 29 percent, respectively). Higher one-year hospitalization rates were seen for patients with misdiagnosis (54 versus 33 percent)."Eczema emerged as a notable mimic, possibly due to similar distributions and morphologic features," the authors write.One author disclosed ties to the biopharmaceutical industry.

WEDNESDAY, Feb. 19, 2025 (HealthDay News) -- Formulation characteristics can explain much of the observed variation in benzene levels of over-the-counter benzoyl peroxide-containing products, according to a research letter published online Feb. 12 in JAMA Dermatology.John S. Barbieri, M.D., from Brigham and Women's Hospital in Boston, and colleagues examined the association of formulation with benzene levels of over-the-counter benzoyl peroxide-containing products. The vehicles for 111 products that had their benzene levels examined were evaluated.The researchers found that leave-on products were associated with significantly lower concentrations of benzene (coefficient, −5.83). No significant association was seen between benzoyl peroxide concentration and benzene concentrations. A weak, but statistically significant, association was seen between days to expiration date and benzene concentration (coefficient, −0.010). Conditions consistent with hot processing were associated with increased benzene concentrations (coefficient, 7.34) in multivariable regression models adjusted for benzoyl peroxide concentration, product type, and days until expiration date. Butylated hydroxytoluene was associated with reduced benzene concentrations (coefficient, −8.15). TUESDAY, Jan. 7, 2025 (HealthDay News) -- Videoconferencing and the application of filters are significantly associated with a heightened interest in cosmetic procedures, according to a study published online Jan. 6 in the Journal of Clinical and Aesthetic Dermatology.Payal M. Patel, M.D., from Massachusetts General Hospital in Boston, and colleagues examined the association between the increased use of videoconferencing and image-enhancing software filters during the COVID-19 pandemic and the corresponding rise in interest in cosmetic procedures. The analysis included survey results from 505 U.S. adults.The researchers found that videoconferencing was frequent, with 88.6 percent using it more than three days a week; 68.1 percent reported using "touch-up my appearance" filters more than half the time. The desire for cosmetic procedures was influenced by videoconferencing in 55.9 percent of participants, while 57.8 percent were influenced by filter use. Two-thirds of respondents reported frequent self-viewing, which was strongly associated with interest in cosmetic procedures. Filter use was significantly predicted by higher income (odds ratio, 2.436) and frequent self-viewing behavior (odds ratio, 5.034). 'Videoconferencing Dysmorphia' Drives Interest in Cosmetic Procedures

WEDNESDAY, Nov. 6, 2024 -- Pain is prevalent among Mohs surgeons, most commonly at the neck, upper back, and lower back, according to a study published in the September issue of SKIN.Timothy J. Orlowski, M.D., from the University of Alabama at Birmingham, and colleagues conducted a cross-sectional survey study among members of the American College of Mohs Surgery to examine the prevalence and characteristics of pain and discomfort among trained Mohs surgeons. A total of 107 male and female surgeons responded.The researchers found that chronic pain was reported by 88.7 percent of participants, with the neck, upper back, and lower back the most common anatomic locations for pain (54.2, 38.2, and 33.6 percent, respectively). Compared with female respondents, male respondents less often complained of upper back pain but more often reported lower back pain (odds ratios, 0.349 and 2.462, respectively). Regular pain-relieving medication use was more likely to be reported when participants were suffering from neck or upper back pain (odds ratios, 4.665 and 3.366, respectively). The likelihood of reporting neck pain was lower for respondents regularly using sit-stand stools."Not only are Mohs surgeons at risk for musculoskeletal injury, but Mohs histology technicians and other ancillary staff also remain vulnerable," the authors write. "Future collaboration with current educators, occupational health professionals, and physical and occupational therapists may provide insight on future ergonomic education and modifications."

Pain Prevalent Among Mohs Surgeons, Reported by 88.7 Percent

WEDNESDAY, Oct. 2, 2024 -- The incidence of epidermal necrolysis (EN) is 2.6 cases per million person-years, but overall mortality is 34 percent, according to a study published online Oct. 2 in JAMA Dermatology.Thomas Bettuzzi, M.D., M.P.H., from the Hôpital Henri Mondor in Créteil, France, and colleagues conducted a cohort study using French Health System data from Jan. 1, 2013, to Dec. 31, 2022, to assess the incidence of and compare factors associated with in-hospital and postdischarge mortality among patients with EN. Data were included for 1,221 adult patients with EN.The researchers found that the incidence was 2.6 cases per million person-years. The in-hospital and postdischarge mortality rates were 19 and 15 percent, respectively, for an overall mortality of 34 percent. Factors associated with in-hospital mortality included age (adjusted hazard ratio [AHR], 1.03 per year of age), history of cancer (AHR, 2.04), dementia (AHR, 1.85), liver disease (AHR, 1.81), and EN severity (AHR, 2.14 for toxic epidermal necrolysis versus Stevens-Johnson syndrome). There was no association observed for EN initial severity with mortality after hospital discharge, but the associations for acute complications persisted (AHRs, 2.14 and 2.44 for acute kidney injury and sepsis, respectively). Ophthalmologic and mood disorders were the main sequelae."Despite EN being a rare condition, it is associated with high in-hospital and postdischarge mortality and a high burden of sequelae," the authors write.

Incidence of Epidermal Necrolysis 2.6 Cases Per Million Person-Years

WEDNESDAY, Oct. 2, 2024 -- An artificial intelligence (AI) model, VisualDx, demonstrates diagnostic bias for images in the Fitzpatrick I to III subgroup, according to a study published online Sept. 15 in SKIN.Katrina D. Cirone, M.D., from Western University in London, Ontario, Canada, and colleagues examined whether the AI model VisualDx performs differently when classifying conditions across different skin phenotypes. Sixteen common conditions were included in the image dataset. For each condition, there were three subgroups: "Fitzpatrick I to III," "Fitzpatrick IV to VI," and "Processed," which contained images from the Fitzpatrick I to III subgroup altered to resemble skin with rich pigmentation. A differential diagnosis list was obtained by processing images with VisualDx.The researchers found the highest sensitivity (97 percent) for hidradenitis suppurativa, prurigo nodularis, and tinea versicolor across all subgroups. The lowest sensitivity was seen for atopic dermatitis, postinflammatory hyperpigmentation, and basal cell carcinoma (23, 23, and 27 percent, respectively). For all conditions except acanthosis nigricans, melasma, and melanoma, significantly greater diagnostic sensitivity was noted in the Fitzpatrick I to III subgroup than the Fitzpatrick IV to VI and Processed subgroups. A reduction in sensitivity and specificity was seen for all conditions for the processed images."Future training of AI diagnostic tools should be done using more comprehensive databases so that diagnosis can be accurately performed across all skin phenotypes," the authors write.
