When it comes to treating thicker plaques on hands and feet, why should you consider Zoryve instead of steroids?

You. So one of the nice things about our non steroidal agents, and in particular topical reflumolast, is that it's a pretty small molecule. Now, what's neat about these PDE four s is they actually have to get inside the cell. And to some degree, this can suggest that they can. Obviously, if they can penetrate into a cell, they can actually get through the skin barrier pretty well as well. And obviously, this depends a lot on the formula relation. So that is not operating true by any stretch. But the formulation here is kind of ingenious, and it's formulated in a way that really does get excellent penetration of the skin, including thicker plaques, including thicker areas like hands and feet. And it does so in a way that I think we can see the bioavailability is extremely high. So the drug can get to where it needs to, even in the presence of thick psoriatic plaques or those hands and feet. So that, to me, is a huge, huge issue, because many medicines, if they don't penetrate well, will often have to sort of pretreat. We can use a topical steroid or light therapy or something else, maybe eczema laser. You want to shrink those plaques, get them down, and then you could use something. And I do that a lot. That's what we've had to do. But it's kind of exciting and empowering that maybe for one of the first times, we can say, look, we can potentially even issue using a steroid every time to get the scaling down. And this might actually be enough by itself to do that. On the other hand, I'm more than comfortable using both. And I think that most of my patients, if not all, with psoriasis, are going to have multiple options, and they're going to use different things at different times. So while I think it could be a standalone, particularly for somebody that's maybe had an adverse experience with topical corticosteroids or soberus them in an area, I still think it's something that could be used intermittently as part of a rotational approach, as we do with many other things.
