When changing from an IL-17 to an IL-23 or vice versa, how long do you typically wait before initiating a new drug?

You. Even though clinical trial studies had mandatory washouts of medications, in clinical practice, I generally will switch medications without a washout period if patients are losing efficacy to the previous agent. In fact, I will add on the loading dose if a patient has lost significant efficacy. If patients are switching because of insurance changes or perhaps patient preference and they're doing well, I will just move on to the maintenance dose of that second medication without a washout period.
