What's New in Dermatologic Surgery

In this session, Brett Coldiron, MD, presented a few interesting articles published in Dermatologic Surgery and other journals within the past year. Dr Coldiron began with two articles pertaining to sentinel lymph node biopsies (SLNB) for melanoma. Sentinel lymph node biopsies were found to be unreliable in predicting melanoma mortality in patients under 40 years or older than 60 years of age in one study, while another review article found that wide local excision prior to SLNB does not compromise sentinel lymph node identification. Moving on to perioperative management, Dr Coldiron discussed a recent review of the management of patients undergoing skin surgery while taking direct oral anticoagulants (DOACs). The review article supported the safety of skipping a dose of the DOAC on the day of surgery if there is concern for excessive bleeding. Dr Coldiron concluded this session with an article highlighting the importance of timely radiation therapy for Merkel cell carcinoma. Local and regional recurrence rates at 5 years were significantly increased (28.0% vs 9.2%) for those who had radiation delayed >8 weeks vs £ 8 weeks.
