How to Match into Mohs Surgery and Tips on Becoming a Better Surgical Dermatologist

This series is about helping residents becoming the best they can be in residency and when they graduate while acing their boards. So we're here for you. There's a lot to talk about on this episode. I'm so happy to have as a special guest Dr. William Higgins. He's assistant professor of clinical dermatology and director of the Mos lab at Pennsylvania Hospital. So, Dr. Higgins, how are you? I'm doing great. Thank you so much for having. Really, really looking forward to speaking with you. We have a lot of great topics to discuss before we talk about the board review. Just want to review what we're going to speak about, and it's going to be how to be a better surgical dermatologist as a resident and also how to match into mo surgery, which I know a lot of residents are interested in. So before we discuss that, surgical dermatology is obviously a very important topic. It shows up on the basic exam, has its own core exam, and of course, it's discussed in the boards. So how do you tackle this subject when studying for these exams? I think one of the key tenets is slow and steady wins the race. You don't want to try to cram this one, so having a good plan from the start is important. I actually spoke to some of our wonderful Penn residents tonight and our Moe's fellow and got their input as far as how they studied. But you want to have a good foundational text. One of them mentioned that he used bologna. You could also do something a little easier or bullet point, something like the ASDs surgery primer, which is what I love. Of course, olliecom is a great resource, but use that primary resource and supplement it with other information. You get along the way. And then questions, questions, and more questions. So the AAD question bank, the ASDs rescue app, which I was part of co writing that. So it's near and dear to my heart. And other question banks like the Dermq bank as well, if time permits. And it's a wide range of subjects. I mean, it is focused on dermatologic surgery, but that's everything from tumor biology to cosmetics. So you want to make sure, you want to know your fundamentals, and not just flaps and graphs, but things like tumor biology. The different hpv strains that are associated with certain tumors, different genes that are associated with certain tumors are important to know. And I can't stress how important those are to study before this core exam and your boards. And then, of course, things like suture properties, cautery, surgical instruments, flap repairs. And we say flap repairs. It's more or less like seeing a defect and then seeing it repaired afterwards and being able to identify what flap was done. So being able to identify what a rhombic flap looks like, for example. They love that one because it's confusing. But look for the question mark shape repair, and that's usually going to be a rhombic flap. And then just knowing your key stitches, things like transposition flaps or rhombic flaps, know that you're going to be suturing your secondary defect first before you're suturing your primary defect, as opposed to advancement and rotation, which are sliding flaps, where you're going to be suturing the primary defect first, cosmetic wise. Know your fillers and injectables, and know your lasers, know what wavelengths would be indicated for certain clinical scenarios. Yeah, that's really great advice there. I like what you said about knowing the flap forwards and backwards, because a lot of times we think, here's a defect, what's the flap? But they're going a step further and asking you, here's the flap, what was the defect? Or how did we get to the spot? Or what steps do you have to take to get there? Is that correct? Yeah, they love those questions because unless you've seen it live, it's sort of hard to figure out. So on that note, as well, spend as much time in your local mose unit as possible. We always love when we have residents, fellows, med students rotate with us, and they love that transposition flap that goes from the cheek to the nose, the nasal labial transposition flap, it's confusing unless you see it. So spending time in clinic, seeing how it moves, gives you a real insight as far as what repair to choose. I was also going to make a note of, too. I heard also, there's these AsDs flashcards that can also be very helpful. So to make a plug for that. And I just want to give a shout out to our fellow Harrison Wim, who's gave me some advice on that tonight. And then our wonderful resident who's applying to Moses here, Dan Lewis, as well. Great. Well, thank you to Harrison and Dan. We really appreciate those tips and just wanted to highlight again what you said about slow and steady. Because you come into derm, you have to learn all this medical derm. It seems so daunting, and then all of a sudden, you're like, wait a minute, now I have to learn all this surgical derm. Suture properties and anesthesia, lidocaine, different types of flaps, but it's really slow and steady. And I think if you stick to those resources that you said do a ton of questions, things will work out fine. Yeah. And from what I hear, too, we used to have the in service exam, which was questions that were thrown out, that were put into a test, and so they were just way out of left field. In left field. But from what I hear, the dermatologic surgery Core exam, it's not too outlandish. It's pretty reasonable. Okay. That's nice to hear. So, Dr. Higgins, let's move away from board review and into more practical things. You know, not everybody that's listening is going to be a surgical dermatologist or a mo's surgeon, but everyone's going to have to do excisions and surgery during their residency. And a lot of people who do general derm will do excisions and surgery in their practice, as well. So I want to start out by asking, what are some tips for residents on how to improve their surgical skills in residency when first starting out, and if they've been in residency for a few years, how can they take it to the next level? I love that question. We own excisions. That's our bread and butter right there, whether you're doing mos, whether you're doing general dermatology. And my short answer, which we always say to our residents of Ellos, is, get reps. You got to get your hands dirty to know how to do an excision, a flap, or whatever repair you want to have mastery of. We had a resident recently, well, multiple residents, who were very eager to learn more, and one in particular who wasn't interested in going into mo's, but just really wanted to make sure he honed his skills. So anytime he had a free moment or he didn't have clinic he'd come and join us, and so we'd make sure to get him the opportunity to suture. But as far as tips for excisions to make them go well, just a few things. So trunkle excisions, they're tough because you can get hypertrophic scarring. A couple of ways to avoid that. Plicating sutures. So doing three layered closures where you're plycating the soft tissue or the fascia together to take tension off the top using a long acting suture. I don't think I can use trade names tonight, but sutures that are going to last longer, monofilament sutures that will last longer to prevent that scar spread over time. And I used to be somewhat skeptical about these, but I've become a big fan of the running subcuticular stitch recently, and so I'm a big fan of using that with glue on top. And for older patients who can't reach their back, or of any age for that matter, can't reach the middle of the back, it's a little easier. As far as wound care goes. One other thing I was going to mention as well that I learned is to reverse bevel. So when you're doing an excision on the trunk, instead of making your incision 90 degrees with the skin, make it so the incision creates epidermal edges that are closer at the top than the subcutaneous tissue at the bottom. So if you imagine you're looking at that cut tissue from the horizon, you'd be seeing epidermal edges that are closer together than the underlying subcutaneous tissue. And that sort of tees you up for a good, nice inverted incision line once it's closed. Yeah, I love what you said there about getting reps. That's the same way I got these big biceps, which I know you can't see because we're on a podcast now. But I have to do a video podcast next time, and I can flex those biceps. But reps is so important the more you do. I was actually a plastic surgeon resident for a couple of years before I switched to derm. So I've seen plastic surgeons and dermatologists do. You know, I've seen some derms do better work than plastic surgeons because they do so many excisions, they really know how to do it well in certain things along those lines. So it's true. Sometimes it's hard, though, to get into the MOS unit or do excisions in clinic. So, at home on your free time, how are some ways that residents can get those reps in and get the practice in. This is kind of funny, but I had almost like a security blanket. I had a pillow I would carry with me in my house. And my wife teases me to this day about this, but I had a pillow that I would take. I would get old surgical tools that were going to be discarded, and I'd clamp either end of the pillow to create two skin edges and I'd stitch on that. But whatever you use, whether it's a pillow, pig's foot, apple, banana, making sure you're doing it consistently, making sure you're coming in with purpose, just sitting there and stitching and doing it without intention is not going to be helpful to you, but come in with a purpose. Are you going to try to work on your buried vertical mattress? Suture? Are you going to try to work on using skin hooks for your deep sutures? Try to use it to hone those things. Do it slowly when you're doing it home because it's going to make you so much better. It's almost like a baseball player with a practice swing. If you do your practice swing well, you're going to hit the ball much better. How do you counsel patients on wound care and scarrings? For example, patient says, this is going to leave no scar, right? Or this is going to leave a minimal scar. What do you tell them? Because I know that's a very important point as well. When doing excisions. That was one of my favorite parts of fellowship, is not only the technical aspects, but learning how to say things in a way that would convey the idea but not scare the patient. So that's a really great question. I always say that whoever tells you there's not going to be a scar might be fibbing. There will always be a scar. Some of my key lines, my Higgins isms, if you will, that I'm sure my nurses would, if I had a pull cord on my back. These are things I would say all the time. But telling patients like an expected timeline is helpful. My fellowship director used to say, well, it takes your mother, it takes our mothers nine to ten months to make our skin, so we have to expect some time to heal. And I love that line because patients will expect a week or two and it's going to be back to normal. I always say it's going to be pink and bumpy for three to four months. So pink and bumpy. And I'll even quiz them before they leave on that and letting them know that that time course is so helpful. You're going to put them at ease. Also letting them know that you're not leaving them out in the breeze. If I do a flap or larger pair in a patient, I'll always say, hey, the door is open. I'd love to see you in four months, especially when you're first out. That is how you're going to really hone your skill. Looking at that repair and thinking, oh, yeah, I remember I did that tip stitch there. Oh, I remember. I really tried to really avert that edge. That's how you're going to get that feedback that's going to make you so much better and to grow. But I think that's a great question and patients really appreciate that when you tell them it up front. Right there's the art of medicine, the science of medicine. That's where the art comes in. Everyone should be on the lookout for the Dr. Higgins doll this holiday season, where you pull the string and pink and bumpy. Pink and bumpy will be repeated. That's going to hit the stores pretty soon. So, Dr. Higgins, this has been great information, but I want to take a moment and pause here and let our listeners know about a really exciting opportunity for residents. That's coming up. So, next, I want to ask about matching into mos, which some people might not be interested in, but I know a lot of residents are. And I looked at the numbers just the other day, getting ready for the episode, and I saw that 50% of applicants match into Mos. And these aren't just people that you see at the bus stop. These are all high performance. Bring intelligent dermatology residents. So please give me some tips. Give all the residents some tips about how we start this journey. If we're interested in matching to Mos. Yeah, it has become I will say this, I'm glad I'm not applying now. I feel fortunate to have matched. It's very competitive. I review applications and it is so hard to determine who to interview, who not to interview because everyone is just excellent. That being said, what we're looking for is a narrative in their application we're looking for. Oftentimes it's not always the case, but some sort of thread of interest that may have started in their first year, their second year. But where they have some publications they may have done like a case report their first year. In their second year they may have done a case series. In the third year they may have designed a larger study. Seeing a thread of activity is important. It's very hard to see an application where they had fantastic board scores. They have a lot of publications, but nothing relates to mos. Unfortunately, what we intuit about that is that maybe they just made a last minute decision to do Mos. So we want to see a narrative in their application. Good letters of recommendation. I love unique letters, unique things like this is the person I want to bring home and have my daughter marry. Type of comments that set applicants apart. But a letter writer who knows you well and isn't just going to regurgitate your applicants. There's three rounds for the Penn Mose Fellowship and the second round we have applicants send in a video of them suturing so you don't have to be God's gift to surgery yet. But having some basic skills, smooth, consistent technique is important and also a willingness to accept feedback. I will also say the analogy I often give our applicants is, and this may sound silly, but it is somewhat of a dating game. The programs want to see an interest coming from the applicants, and of course, vice versa. So showing that program that you're interested is very helpful, and I call that being politely persistent. But say you apply, you don't hear back. Maybe an email with a summary of like hey, since we last corresponded, I published this paper on Merkel cell carcinoma and had these two abstracts accepted. Fellowship directors aren't bothered by that. They may not answered, but they're not bothered by it. And it's a way to continue to put yourself on the radar to make sure you're noticed and that your name gets out there. Going to meetings is helpful. The ASDs always happens to fall right after the applications go in, so I always tell our applicants to try to go there, show your face, and that's a real nice way to introduce yourself and often get a few extra interviews just from some face to face contact. Yeah. Mo is, as you mentioned, extremely competitive. But you know what? So is dermatology. And everyone made it into dermatology with determination. So if you have determination, you'll make it there, too. What about applicants or residents who don't have a MOS program or don't have a really strong MOS program? How can they get some more research and some more opportunities as a resident if they're interested in doing MOS? So we see that not infrequently. Mentorship is very key, and finding a mentor who's. You don't typically want somebody who's like, 2030 years out. You want somebody who's kind of maybe five to ten years out, somewhat familiar with the process, who can help navigate how you're going to approach your interviews, et cetera. And you want to reach out to either at a meeting or via email, just people you think might have shared interest. I have mentors that I approached five or six times before I got traction, and now we speak every couple of weeks. But again, be politely persistent. So if you're in a program where you don't have a MOS fellowship, reach out, see if there's a research project you can get on, see if you can rotate it at an institution, because there are a lot of people out there that are willing to be your advocate. And then you're going to also need for those letters to be written to have a strong support of your application. Excellent. Well, Dr. Higgins, we're almost out of time, but I have one more question for you, and this is the most important question. I love food, all kinds of food, but I especially love cheesesteaks. And I know you practice in Philadelphia, so I want to ask you, where is your favorite spot to get a know Nick? My nerves. So, prior to coming here, I was in New England, so I was calling them grinders and all sorts of things that are sacrilege in the city of Philadelphia. My nurses at the Pennsylvania hospital mose unit heard that I hadn't had a cheesesteak, and so they said, we got to get you one. So one day, they brought me Jim's cheesesteak, and that's the only one I've tried, and so that's my favorite, and it was delicious. Yeah. So Jim's would be my answer. Yeah. Are you a wiz wit guy, or how do you take the cheesesteak? You know, I had it with wiz, and I liked it, so I'll have to say yes with wiz. All right. Cheesesteaks are great. I've been very, very fortunate to be part of a lot of activities and being involved with Mark and Darryl and now several others. With the fall clinical and winter clinical dermatology meetings, it's expanded, and it's included these derms and conditions podcasts. We just released this week, the 24th episode, and it's been extremely successful. Very proud of it. Along the way comes Nick Brownstone is one of Darryl's fellows, and Darryl has had a series of fellows, all of them been very successful. Nick is extremely creative and comes up with many, many great ideas, and one of them was to have a podcast for residents. So my role here as the host extraordinaire is to pass that baton over to Nick for this new group of podcasts that he is going to be organizing and serving as the host. So Nick deserves a great round of applause. He's done a great job. So we talked, and we thought, who else should we get to be the first guest? And it's a pleasure to have Mark Kaufman here. Mark is a great guy, excellent dermatologist, has done a lot in dermatology and worked his way up to now being recognized and voted in as president of the American Academy of Dermatology. So, Nick, I'm going to turn it over to you. And welcome, everybody, to cutaneous miscellaneous. He was seeing a groundbreaking event, the very first one. Thank you, Dr. Del Rosso. It's really an honor to serve as the host of this new podcast. As we all know, there's not a ton of information and resources out there for residents. So you wanted to make a podcast series with the goal of maximizing your potential as a dermatology resident while maximizing your board scores. I had a mentor tell me a while ago that as you move through dermatology residency, the hard stuff becomes soft. And the soft stuff becomes hard. Towards the end of your third year, you could probably diagnose most rashes, treat most skin cancers, and give Botox like a pro. But the softer stuff, like negotiating your first contract, giving an impactful podium presentation, dealing with difficult office staff, and learning the do's and don'ts of working in the industry becomes a little more difficult. This podcast is going to help you with some of those questions and related issues, which is why we're here today. This podcast is talking about how the AAD can help jumpstart your career and who better to have as a panelist, Dr. Mark Kaufman, the newly inaugurated president. So, Dr. Kaufman, thank you for being here. I'd love to get us started with our first question, which is, in a nutshell, what does the Aad do and how does it help residents and those graduating residency? Well, thanks for that question, Nick. And thank you, Jim and Samal, for the introduction. It's great to be here. I'm pleased to tell everyone I survived my first week as president of the academy. I don't know how much longer I can say that. So it's a pleasure to be here. The academy. What I'm going to do is just introduce the academy to you in general and tell you where the academy wants to really work with residents. The four areas of focus of the AAD. The first one, the first pillar, the biggest pillar is education. Right? And we just had a huge AAD meeting. A lot of you were probably there. It was very successful. Over 15,000 people at the meeting. There were courses that were there for residents. Right. In preparing for the boards, in preparing for business boot camp, all of these things to try and get you guys more prepared. The academy is very interested in your transition from being a resident to being a practicing dermatologist. And what we want to do is prepare you with lots of materials, but we also want you to be members when you're residents. You guys probably all know you get complimentary membership in the academy as a resident and as a fellow. There's a $500 stipend to go to the AAD meeting. You get the Blue journal and dialogues and dermatology included in your membership. So there are a lot of things that you're getting from the academy to try and prepare you both educationally. And when it comes to that transition to the next step, the next area of focus I'll touch on is advocacy. Advocacy is very close to my heart because I actually have come up through the academy through its advocacy arm. I've been on our ruck team, which none of you probably or some of you may have heard of. It's where we go and fight for the value of our procedures, and it's very important for us as an academy to be able to go to Washington and go to legislators and lobby them for our needs. And we do that every year in person. This year, again, for the first time in a few years, the legislative conference, the academy's legislative conference is going to be on September 18. And residents are usually given, if you apply for it, scholarship to come to that meeting. Totally covered. Okay. I would try and urge all of you to think about coming to the legislative conference. You'll learn a lot. It's not just about going to the Hill, but it's all the stuff you learn the days before that. So it's really important. The third element is public relations. We need to tell the public how good we are, the primacy of the board certified dermatologists. Right. And that's what we're going to do. And you're hopefully going to see more social media stuff. And I think they'll be very reassured to hear that it won't be you and me running the social media campaign, Jim, but we're going to have hopefully more in social media that you guys are much better at than we are here. And then the fourth pillar is research. And a lot of you do research, want to continue to do research. And we advocate for those research dollars. And that's really, in a nutshell, what the academy is focused on. We've had a great discussion so far, but let's pause and take a moment and hear from our sponsor. So how can the academy help residents and those about to transition into practice? So a lot of the stuff is what I already mentioned, the materials that are available to you. But the other thing and the best way for you guys to all get involved in the academy is actually to step up. And it happens to be coincidentally, this is the time. Okay. On April 1, you all should have gotten an email from Dr. Terry Cronin, who's the president elect. He'll be president after me. And it basically told you how to apply for committees in the organization in the academy. And it's really important for you to go find that email to look at the list of committees that are available and to apply for the ones that you want to get involved with. There is a committee, there's a residents and fellows committee which is completely dedicated to actually resident and fellows issues. And the chair of that committee actually sits on the board of directors as a board observer. So it sounds like it's going to be impossible to get involved as a resident but we're trying to make it easier and easier to get involved. And every one of the committees, virtually every single one of our committees has a spot that's dedicated to a resident or a fellow. And so you can apply. If you want to be on the council of Communications, you can apply to be the resident member of the council of Communications. So the stuff that interests you should be the stuff that you apply for. And then the most important thing to do after that is find one of your mentors, one of your residency directors, and have them write a letter of recommendation for you to send to Dr. Cronin. Because having that letter makes it just a lot easier for us to place you on those committees. Mark, I have a question for you. Imagine I'm a resident. It's been a long time, but I would be know I'd like to get involved, but I'm concerned how much time it's going to take. I'm just going to be getting out there. I'm looking for a job. I have to move. I got to take my boards. There's all these pressures, and I don't want to jump into something where I might be biting off more than I can chew, time wise. What would you suggest with regard to that? I couldn't do it because I couldn't find anybody to write me a letter recommendation, but they probably won't have that difficulty. I'll write you one the time concern. Nick will write me one the time concern. I'm hoping, Jim, that you're going to apply this round, but it is not as time consuming as one might think. There will be some quarterly conference calls. You'll meet in person at the annual meeting and sometimes at the summer meeting, but it really is as much effort as you're going to put in. So if you really want to get involved, do it. And if you just want to see what it's like, dip your toes in it, then do that and see if this is something you want to do. And if not, you can apply the next year for a completely different committee. You can only get appointed to one committee a year, but you can get one every single year, and that goes for when you get out as well. That's not just when you're a resident, but you can apply for all these committees. And the more committees you're on, the more people are going to see you. And if I could just give one piece of advice to you, it's to allow opportunities to evolve. Never think that you know exactly where you're going to be in 20 years. Because I can tell you complete honesty with complete honesty, that if anyone would have told me I'd be here as president of the academy 20 years ago, I would have thought you were insane. Okay? But opportunities happen, and you should let them evolve. And even if it's going in a direction that was completely different than what you thought you were going to do. Explore, keep an open mind. And I think you'll find and find those mentors. That's the most important thing. Find the faculty members, find the colleagues that you want to say. I want to be like him or her. That's the most important thing for you to do. As residents. You mentioned a lot of committees. Can you give some more examples of committees that residents can serve on? They can serve on most every committee. The residents and fellow committee is probably the toughest one to get on because every resident and fellow wants to be on that committee. But there are plenty of committees that residents can sit on. And when you get out, there's also a young physicians committee that you can join and continue. And the young Physicians Committee also has a board observer on the board of directors. So there's a lot of ways to get involved. In the email from Dr. Cronin, there'll be a list of the committees that have openings this year. So just find one that you like. Don't apply for just one. Apply for, like three, because we can't guarantee that we can get you on one committee, but we're certainly going to try and get you on a committee. Thank you, Dr. Kaufman.
