Cutaneous lupus is seen as a dermatologic disease, but not always a dermatology-only disease. In this topical conversation, Saakshi Khattri, MD and Joseph Merola, MD, MMSc, discuss how dermatology clinicians can approach CLE with more confidence, from distinguishing skin-limited disease from presentations that warrant systemic evaluation, to knowing when rheumatology should enter the conversation.

The discussion moves beyond recognition alone and into the decisions that shape long-term outcomes: what to ask at the first visit, which baseline labs may help risk-stratify patients, why urinalysis should not be overlooked, and when active skin disease should prompt earlier escalation. Throughout, the conversation returns to a central point for dermatology—CLE is not “just skin.” It is a potentially scarring disease in which delayed or undertreated inflammation can leave patients with pigmentary change, alopecia, and irreversible damage.
