How important are vehicles in topical Plaque Psoriasis therapy?

As Dr. Leon Kersick likes to say, vehicles matter. They're critically important for all topical therapy. Vehicles determine penetration, how much of the drug gets through the stratum corneum and permeation, how much of the drug stays in the skin and both of those contribute to efficacy. To that end, a topical that I frequently used because of its rationally designed vehicle which was formulated without propylene glycol and with an eye to enhanced penetration and permeation is sernivo, which is a 0.5% betamethasone dipropionate spray classified as medium potency but with data to show efficacy on par with augmented betamethasone lotion, a high potency steroid. Moving beyond efficacy, vehicles contribute to absorption and in the case of topical corticosteroids, absorption could contribute to HPA axis suppression and therefore are critical for safety. And lastly, vehicles can be an avenue for other side effects like stinging, burning, allergy, irritation and these things affect compliance.
