How do I transition from cyclosporine to dupilumab for patients with atopic dermatitis?

When a patient is on cyclosporin and then he or she then gets approved for dupilamab. Typically I would add dupilamab while the patient is still on the current dose of cyclosporin. I would wait until the patient is a lot better clear or almost clear or pretty much very mild disease on both a combination of dupilumab as well as cyclosporin. Once they are clear or almost clear or really extremely mild, then I would taper off cyclosporin from bid dosing to QD dosing for about two weeks and if they're still doing well and then go from QD dosing to QOD dosing for another two weeks. And hopefully by then you can then after that have patient come off of cyclosporin completely while they're being managed on dupilumab.
