What patient and disease characteristics do you consider when choosing a systemic therapy for patients with metastatic SCC?

You, patients with metastatic squamous cell carcinoma can be very challenging to manage. I think the most challenging group are the solid organ transplant patients. So we know that we need to balance trying to protect their engrafted organ, making sure that we don't do something that risks rejection, particularly in something like lung transplant or a liver transplant. So that often takes immunotherapy off the table. The other factors that we need to think about are, where is their metastasis? So, for example, in a patient who's not a transplant patient, who has lymph node metastases, one of the considerations that we have to think about is, should we be thinking about neoadjuvant therapy? So treating them and then having surgery, or, or do we want to go right to surgery? We also need to think about the patient's goals of care and also the patient's performance status, make sure that we give them treatments that they can tolerate.
